THOMAS B. PUSCHAK, DMDDr. John Smith was born and raised in Minnesota. He received his degree from the University of Minnesota in 1990. He practiced in Minneapolis for ten years. Dr. Smith entered the Master'Degree program in at the University of Minnesota in 2000. Dr Smith enjoys spending time with his family, traveling, and exploring the wilderness of the Boundary Waters. He is a talented musician and avid reader.

Carol – Office Manager
Carol began working for Family Dentistry in 1989 with Dr. Puschak as his dental assistant and office manager. She has taken a major part in the growth of the practice and has continued to provide the best care to all of her patients.

Anna – Dental Hygienist
Anna is a graduate of the Forsyth Institute and has been a dental hygienist for 10 years and performs periodontal scaling treatments. She is bilingual and speaks Russian. Anna spends time raising her kids and enjoys ballroom dancing.

Molly – Dental Hygienist
Molly is a graduate of the Forsyth Institute. In addition to dental cleanings, she performs periodontal scaling treatments.
